Neck pain remains as a major cause of disabilty, according to Dr. A. J. Farshchian MD the medical director for the center for Regenerative medicine.
All the interconnected structures that give your neck its incredible range of motion are subject to the wear-and-tear damage of arthritis and the overextension injuries of whiplash. Neck pain may result from abnormalities in the soft tissues – the muscles, ligaments, and nerves – as well as in bones and joints of the spine. The most common causes of neck pain are soft tissue abnormalities due to injury or prolonged wear and tear. In rare cases, infection or tumors may cause neck pain. In some people, neck problems may be the source of pain in the upper back, shoulders or arms.
Degenerative diseases that cause neck pain include osteoarthritis and rheumatoid arthritis. Osteoarthritis usually occurs in older people as a result of wear of the joints between the bones in the neck. Rheumatoid arthritis can cause destruction of the joints of the neck. Both of these major types of arthritis can cause stiffness and pain.
Cervical disk degeneration also can cause neck pain. The disk acts as a shock absorber between the bones in the neck. In cervical disk degeneration (typically age 40 onwards), the normal gelatin-like center of the disk degenerates and the space between the vertebrae narrows. As the disk space narrows, added stress is applied to the joints of the spine causing further wear and degenerative disease.
The cervical disk may also protrude and cause pressure on the spinal cord or nerve roots when the rim of the disk weakens. This is known as a herniated cervical disk. Because the neck is so flexible and because it supports the head, it is extremely vulnerable to injury. Motor vehicle or diving accidents, contact sports, and falls may result in neck injury. The regular use of safety belts in motor vehicles can help to prevent or minimize injury. A “rear end” automobile collision may result in hyper extension, a backward motion of the neck beyond normal limits, or hyperflexion, a forward motion of the neck beyond normal limits. Most common injuries are to the soft tissues, i.e., muscles and ligaments. Severe injury with fracture or dislocation of the neck may damage the spinal cord and cause paralysis .
Much less common causes of neck pain include tumors, infections, or congenital abnormalities of the vertebrae. There is also the emotional part, I mean why would the pain get worse with stress and get better with less stress, this must be emphasized when treating patient with neck pain other wise the job is not done. Why the tingling of the fingers? this is because of nerve conduction and dermatomes. Dermatomes are located on the anterior body and the posterior body and are consecutive in the neck and torso regions. Development of the peripheral nervous system will produce a pattern of skin innervated by cutaneous neurons of a certain spinal or cranial nerves. These are called dermatomes and represent specific regions of nerve reception of sensory impulses. The pattern of the dermatome is of major clinical significance when a physician desires to treat a particular portion of the body. For instance the gentleman discussed in the above case having numbness in the index finger as well as his thumb (refer to the picture) may need to be treated specifically at the level of C6-C7.

National Academy of Elder Law Attorneys Kicks Off District Days 2012

With their eyes on a critical election this fall, members of Congress are frequently returning home to engage with voters in their districts. After several years of successfully lobbying Congress in Washington, D.C., with an annual “Hill Day,” the National Academy of Elder Law Attorneys (NAELA) is taking a grassroots approach with District Days, an advocacy event to educate members of Congress when they are in their home offices on issues of importance to older Americans and individuals with special needs.
NAELA’s District Days launched in May, when representatives and senators returned home for a week-long recess. NAELA members met with their legislators and shared their ideas and concerns on the federal policy issues that affect their clients and practice, including:
Support for reauthorization of the Older Americans Act, which provides older adults with much-needed services that include home care, congregate and home-delivered meals, case management, caregiver support, legal services, transportation, elder abuse prevention and job training, and the long-term care ombudsman program;
Opposition to efforts to convert the Medicare program into a premium support system, a model that would replace Medicare’s current guarantee of coverage with a premium-support voucher and raise the age of eligibility from 65 to 67;
Opposition to the House budget’s proposed Medicaid block grants, which would restrict funding for state programs, as well as restrict eligibility and funding for long-term services and supports; and
Support for the Disabled Military Protection Act of 2012 (H.R. 4329), which would allow payment of military survivor’s benefits into special needs trusts for disabled children.
NAELA Board member Ron Landsman, CAP, and former NAELA Board member Morris Klein, CELA, CAP, met with Rep. Chris Van Hollen (D-MD), ranking member of the House Budget Committee, as part of NAELA’s District Days advocacy effort. During the meeting, Landsman and Klein asked the congressman to support the Disabled Military Child Protection Act of 2012. Rep. Van Hollen shared his support for the proposal, as well as his concern about inequity if others — civilian government employees and private citizens under Social Security — did not have the same opportunities. Landsman and Klein also discussed the many defects of the so-called Ryan budget proposal, particularly the dismembering of Medicaid federal standards through block grants to states.
NAELA members will continue their advocacy efforts this summer and fall when representatives and senators return home during the weeks of:

September 10-14 (Senate only)
September 24-28
October 8-November 6
As Landsman stated after his visit with Rep. Van Hollen, “Developing relationships with your senators and representatives is important, as we can become valued resources to them and their staff. It is much easier to ask for their support once this relationship has been established.”

In celebration of National Women’s Health Week and in recognition of the numerous accomplishments undertaken at The Feinstein Institute for Medical Research, a newly organized group of female scientists, Advancing Women in Science and Medicine, hosted its first annual awards breakfast on May 17. Scientists, educators, business and community leaders from the region attended the event to celebrate the excellence and achievements of women scientists at the Feinstein Institute.
AWSM was created in an effort to advance the career opportunities and career satisfaction of female scientists at the Feinstein Institute. AWSM is comprised of 25 female faculty members who lead programs in basic, translational and clinical research. The group elected Christine Metz, PhD, as president, and Nadeen Chahine, PhD, as vice president. The four core initiatives of AWSM are in the areas of career development, recognition, advocacy and education, and networking, and mentoring.
“It seems that successful women scientists are less likely to seek recognition for their discoveries,” noted Dr. Metz. “AWSM encourages and assists women in their efforts to be recognized for their achievements, and this annual awards breakfast is a celebration of such achievements. One award of particular esteem is the Scientific Achievement Award, which was conferred to Betty Diamond, MD, head of the Center for Autoimmune and Musculoskeletal Disease at the Feinstein Institute. Lupus is a chronic autoimmune disease that can damage any part of the body (including skin, joints, and/or organs). Texas Dental Team Decides to Put Patients “In the Driver’s Seat” on Amalgam Filling Removal

The dentists at FLOSS Dental have stopped using amalgam fillings for restorations and want patients to make informed decisions on the contentious debate over mercury in amalgam and its potential health impact.
“Look, we know that most of these amalgam fillings will have to come out eventually,” says Clark Steffens, DDS, a FLOSS associate. “As a man-made substance, they can erode and that’s when the mercury gets dangerous.”
The controversy centers on the mercury used in amalgam fillings – and its purported toxic effects on the body. Some research indicates that prolonged exposure to mercury can contribute to neurodegenerative disease, chronic illness and even birth defects. However, most health officials agree that there has not been enough conclusive evidence to warrant the complete elimination of amalgam from the dental industry.
“We want our patients to make informed decisions about the removal of any amalgam fillings,” says Dr Clint Herzog, CEO of FLOSS. “If a patient wants the amalgam removed, we can certainly do it. However, dental amalgam has shown to be an effective material with some real benefits – and there’s still a place in dentistry for a well-placed amalgam filling when no better alternative is available.”
One of the most popular alternatives to amalgam is composite resin. It’s made of plastic reinforced with powdered glass. It’s environmentally safe and doesn’t require the removal of as much tooth structure as amalgam in most instances. Already, about half of U.S. dentists are mercury-free and a majority of patients, if given a choice, are electing mercury-free alternatives.
“We have simply chosen to avoid the potential risks altogether by using mercury-free alternatives in any situation where it is clinically acceptable to do so,” says Herzog. FLOSS has more than a dozen dental practices across Texas, where dental teams remove failed amalgam fillings daily, often replacing them with composite resin.
For patients who currently have amalgam fillings in good condition with no decay, the FLOSS Dental team agrees with the FDA and the American Dental Association in that immediate removal or replacement is unnecessary.
“It’s important to understand, though, that failing amalgam restorations can pose real health risks regardless of the mercury content dilemma,” says Justin Ramsey, DDS, and a FLOSS associate. “…including tooth decay, fracture and eventual tooth loss or systemic infection if left untreated.”
For several years, the FDA has vacillated its position on mercury exposure related to dental amalgam restorations, always adopting a fairly neutral position. According to the World Health Organization (WHO), however, there is no harmless level of mercury.
Physicians and scientists for the WHO have studied the effects of mercury fillings for many years and have indicated that dental amalgam contributes significantly to mercury levels in the human body.
“The real decision-maker here is the patient,” says Dr. Herzog. “Without a unanimous position from the government or the dental community, we try to provide every patient with the latest research and help them make an informed decision on their oral and overall health.”
